MLTC (Managed Long-Term Care) is a program that coordinates long-term care services for those with chronic conditions or disabilities, helping cover the cost of adult day care and related support.
A Social Adult Day Care Program (SADC) offers structured, comprehensive day services for functionally impaired adults in a protective, supportive, and socially engaging setting. Programs like ours promote community-based long-term care, helping participants remain safely at home while receiving the care they need during the day. Services include: • Socialization and supervised recreation • Personal care and assistance with Activities of Daily Living (ADLs) • Health and nutrition support • Cognitive and physical stimulation • Transportation and caregiver respite
We are on Northern Boulevard, minutes from the 7 train and served by several bus routes. Free door-to-door transport is available for enrolled members.

For more than 40 years, M.A. Aziz has dedicated his career to building stronger communities. As a respected leader in the construction and real estate development industry, he has developed more than 100 affordable housing units, providing safe and quality homes for low-income families and senior citizens throughout New York.
Throughout his career, Mr. Aziz believed that building homes was only one part of serving the community. He understood that seniors deserve not only a place to live but also compassionate care, dignity, and meaningful opportunities to remain active and connected.
With this vision, he established Your Dream Home Care in October 2022 to provide high-quality home care services that help older adults live independently and comfortably in their own homes.
Mr. Aziz’s commitment to public service extends far beyond his business accomplishments. He has been elected President of the Bangladesh Society twice and has served as a Member of the Board of Trustees of the Bangladesh Society for more than 13 years. Through these leadership roles, he has continuously supported families, seniors, and community initiatives.
Inspired by a lifelong passion for helping others, Mr. Aziz founded RAFA Adult Day Care to provide a warm, welcoming, and supportive environment where older adults can enjoy companionship, engaging activities, nutritious meals, wellness programs, and compassionate care. His vision is to create a place where every member feels respected, valued, safe, and connected.
